Series introduction: In 1984 the Institute of Marine Engineers published a paper by LR Colleagues R. F. Munro and P. E. Haynes on machinery failures and their causes. The paper provided 30 case studies of failures in steam turbines, diesel engines, shafting and propellors, engine room, fires and flooding. Failures were grouped into in two categories, operational failure from crew mistakes and design failures primarily due to fatigue failures in reciprocating components. In discussion with the Institute, LR has agreed to repeat the paper and initiate a series of webinars on ship and equipment failures 40 years later. LR will draw on its archive of accidents and incidents plus the work of the Technical Failure and Investigation team to initiate a programme to which other organisations are invited to contribute. We can reflect on whether things have changed in the intervening 40 years and build a similar body of knowledge to support the next generation of marine engineers.
Session: The first session looked at the findings of that original paper and some of the recent major incidents experienced in the marine industry and their causes related to machinery failures, supply chain issues and the impacts of software and systems. This session will look at the work of LR’s Technical and Failure Investigation Department giving a brief overview of the TFI global team and capabilities, laboratory facilities and then some interesting case studies related to ship machinery failures investigations they have been involved with in recent years.
