In 1984 the Institute of Marine Engineers published a paper by R. F. Munro and P. E. Haynes of Lloyd's Register (LR) on machinery failures and their causes. The paper provided 30 case studies of failures in steam turbines, diesel engines, shafting and propellors, engine rooms, fires and flooding. Failures were grouped into in two categories, operational failure from crew mistakes and design failures primarily due to fatigue failures in reciprocating components. LR has kindly agreed to repeat the paper and initiate a series of webinars on ship and equipment failures 40 years later and it will draw on its archive of accidents and incidents plus the work of the Technical Failure and Investigation team to initiate a programme to which other organsiations are invited to contribute.

This first session will look at the findings of that original paper and some of the recent major incidents experienced in the industry and their causes. Future sessions will look at recent machinery failures, supply chain issues and the impacts of software and systems. We aim to reflect on whether things have changed in the intervening 40 years and build a similar body of knowledge to support the next generation of marine engineers.
